1. Request Forms
Please download the appropriate "Request Form" from below. For those who cannot download the forms for any reason, VANTEC will send printed versions of the forms by mail.

2. Enclosing Items
Please enclose the following two items when posting the "Request Form".
- Identity verification documents (please see below for details)
About "Identity Verification Documents" - a postal money order for service fee (this is not necessary for requests regarding correction, addition, deletion, disabling use of, erasure, or discontinuance of third party usage of personal information.)
3.Service Fee Payment
For requests to receive disclosure and notice regarding the purpose of use of personal information, we charge a service fee of 800 yen. Please purchase a postal money order in the amount of 800 yen, and send it with enclosing documents to VANTEC. The purchase cost of the postal money order and postage to VANTEC will be burdened by a sender.
VANTEC will not refund this service fee even if we cannot disclose personal information, or notify the purpose of use, for reasons of not holding the information in question, or due to reasons stipulated by law.
Please contact the "Office for inquiries regarding personal information protection" when paying the service fee by means of a bank transfer.
If the service fee duly received is less than our requested amount of 800 yen, or the money order is not enclosed in the envelope, we will send a further notice requesting a full complete payment to the sender. Then, if the payment is not made within a certain period of time, VANTEC will deem that no request for disclosure or notice regarding the purpose of use of personal information has been made.
We do not charge service fee for requests such as correction, addition, deletion, disabling use of, erasure, or discontinuance of third party usage of personal information.
4. Mailing Method
Please send us the following materials 1 and 2 and 3 to the billing address noted below (5) by simplified registered mail or acceptance-recorded mail.
(The purchase cost of the postal money order and postage to VANTEC will be borne by a sender.)

6. Our Reply
We will send our replies via restricted delivery mail (special delivery) to the address written on the identity verification document sent to us. A period of 2 weeks will be required to look into the inquiry and subsequently send a reply. If for any reason we require more than 2 weeks, we will notify the person as such.
